Records Release Form


LOAN POLICY
  1. All records on loan must be listed on the Records Release Form.
  2. Departmental records may be loaned out to AFL-CIO personnel for office use only. Generally, departmental records will be released only to the office of origin.
  3. No processed records, unique reference material, or microfilm will be loaned out. Rather, the GMMA will provide reproductions of such records.
  4. All records are to be returned to storage within one month of the date of their release, unless specifically requested and released for a longer period.
All fields must be completed, unless otherwise noted. If you have more than three requests, please fill out and submit an additional form.

[Note: For accession (e.g., AR2002-0000), box, and locator (e.g., B01/A/01) numbers, refer to the Records Transfer Inventory (RTI) form that you retain in a master file.]

After filling out this form, print a copy for your records and click the "submit" button below. GMMA staff will generate a hard copy version of this form to accompany the records, and will send an email to you when they are en route. When you receive the records, notify the GMMA by return email. The staff person signing that form assumes responsibility for care of the records on loan, and assures their return to storage.

There is no delivery charge for AFL-CIO departments and offices; however, AFL-CIO trade and industrial departments and allied/constituency groups will be billed for delivery of boxes via the AFL-CIO's messenger service.
